This notice applies to protected health information ("PHI") handled by independently licensed physicians and licensed pharmacies associated with Pepton Health's platform, operated on HIPAA-compliant infrastructure provided by Rimo Health.

2. How We Use and Disclose PHI

We use and disclose PHI for:

Treatment: Physicians review your intake to determine appropriate care; pharmacies fulfill prescriptions
Payment: To bill you or your designated payer
Healthcare operations: Quality assessment, credentialing, and administrative activities
As required by law: Public health, law enforcement, and similar legal obligations

3. Other Uses Require Your Authorization

Uses and disclosures not described above will be made only with your written authorization. You may revoke authorization at any time in writing.

4. Your Rights

Access and receive a copy of your PHI
Request correction of your PHI
Receive an accounting of disclosures
Request restrictions on use or disclosure
Request confidential communications
Receive a paper copy of this notice upon request

5. Complaints

If you believe your privacy rights have been violated, you may file a complaint with us at privacy@peptonhealth.com or with the US Department of Health and Human Services. You will not be retaliated against for filing a complaint.
